Health and Welfare Benefits Resources

Eligibility
Must be a permanent resident of Ontario maintaining a primary residence in Ontario and residing there at least 153 per year.

Coverage
18 months
After age 65, OHIP covers one eye exam and 2 minor follow-up assessments every 18 months

Eligibility
65 years or older
-OR-
living in a long-term care home, home for special care, Community Home for opportunity; 24 years of age or younger and not covered by a private insurance plan; receiving professional home and community care services; receiving benefits from Ontario Works or Ontario Disability Support Program; enrolled in the Trillium Drug Program

Coverage
Annual $100 per person deductible; copayment up to $6.11 per eligible prescription.
Seniors Co-Payment Plan for low-income seniors (net annual income of $25,000 for a single senior or $41,500 for a senior couple) – no annual deductible; reduced co-payment of $2 per eligible prescription.
If living in long-term care, home for special care, or community home for opportunity, receiving professional home and community care services, or receiving benefits from Ontario Works or Ontario Disability Support Program (ODSP) – no deductible; reduced co-payment of up to $2 per eligible prescription.

Eligibility

Resident of Ontario with valid Ontario health card number; do not already qualify for the Ontario Drug Benefit program (for example, you are not enrolled in a program such as Ontario Works); do not have an insurance plan that pays for 100% of your drugs; spend about 4% or more of your after-tax household income on prescriptiondrug costs

Coverage
Annual deductible (4% of net income, paid quarterly); $2 copayment per prescription

Eligibility
65 years of age or older; resident of Ontario; annual net income of $25,000 or less for a single senior OR a combined annual net income of $41,500 or less for a couple; no other form of dental benefits, apart from the Canadian Dental Care Plan (CDCP), including private insurance or dental coverage under another government program

Coverage

  • check-ups, including scaling, fluoride and polishing
  • repairing broken teeth and cavities
  • x-rays
  • removing teeth or abnormal tissue (oral surgery)
  • anesthesia
  • treating infection and pain (endodontic services)
  • treating gum conditions and diseases (periodontal services)
  • partial coverage of dentures

No access to dental insurance Adjusted family net income is less $90,000

Coordination of coverage with OSDCP

Eligibility
Resident of Ontario with valid Ontario health card; have a disability requiring the equipment or supplies for 6 months or longer Cannot qualify or receive financial support for same equipment/supplies through WSIB or Veterans Affairs Canada

ADP billed directly for 75% of cost of the following equipment and supplies:

For those receiving financial support from Ontario Works, ODSP, or Assistance for Children with Severe Disabilities, ADP covers 100% of cost
