Do you suspect COVID-19 exposure, but have not been tested?

If this applies to you, please file the WSIB Incident Exposure Form.

Do you have a confirmed case of COVID-19 (tested positive)?

If this applies to you, please file the Worker’s Report – Form 6.

If you have a confirmed case of COVID-19, you should report any exposure to your employer. Also, if you sought medical attention and needed to take time off work, you should request that your employer files a Form 7.

Is your absence from work prolonged due to COVID-19?

If your employer requires you to submit a Functional Abilities Form (FAF), and does not provide you with one to take to your doctor, you can access it by clicking here.

Has your COVID-19 WSIB claim been denied because the WSIB takes the position that the nature of the worker’s employment did not create a risk of contracting the disease beyond that which the public at large is normally exposed?

If so, please contact your union representative immediately and your union will launch an appeal as this is an irrelevant criteria that the WSIB is incorrectly requiring.
